Question about affidavits.

A while ago I read the requirements of what an affidavit/letter must have such as his name and what he does for living.

Someone is making me a affidavit/letter and he will put the time I've known him for. I will like to know what he must include about himself and about me.

The attorney gave me an option which was just a computer letter saying how we met, (he was my boss) since i worked with him and what we did on his job and that we've been in contact since back then he just signed it and on the same letter at the bottom it was a copy of his picture id .
