What you’ll need
If you’re under 18 years:
Show us 1 of the options from the list below:

Option 2: 2 “A-List Documents”

Your parent/guardian must come with you to sign the affidavit.

All documents required to prove their identity. (Their Washington state driver license or U.S. passport.)
Proof that they’re your parent/guardian. (Your certified birth certificate, adoption papers, or a school transcript with their name on it.)
If your last name is different than your parent’s or guardian’s, or if their proof of identity is an out-of-state driver license and they don’t have a Social Security number, we’ll require more documents (A divorce decree or marriage certificate).

Without SSN, he need to show proof of residence. School transcript should work. If he can't obtain any docs from school, tell him to open a bank account. They take bank statement as a proof of residence too.

You have to sign a declaration and show proof of residence if you don't have an SSN.

Your cousin is good since his father has a WA license.

This only apply to people who have a out of state driver license.

ID should be fine. If he is worry, tell him to bring a divorce decree or marriage certificate with him.
