Ok guys so these what am sending as proof,

Proof of identity
a. Passport
Proof of arriaval before age 16
a. Expired passport with immigration stamp
b. expired visa stamp
c. Expired i-94
Proof of continues residency of 5 years.
a.Bank statements from 05/16/2007-06/15/2007
b.Credit card statements from 06/01/2007-10/01/2007
c.Bank statements from 09/04/2007-present
d.Car insurance from 11/01/2010-present
e.Lease rental from 05/01/2011-present
Proof of high school graduation
a.high school diploma
Proof of fisical presence on 06/15/2012
a.bank statement for june 2012

If possible and you came here lets say in 1999 or 2000 (for instance) try to provide something for each year if you can. I submitted a bunch of certificates from grade school. Are you in college? Maybe show a letter from there if you can.

Dont forget the money order and make sure you sign all of the signs in the proper places.

Looks good, are you adding a cover letter?

It's not a must, I wasn't even thinking of sending one myself, but just to be a little more organized and cautious i did one to make the person reviewing my case understand exactly what every document proved.

I wonder how it went with this person

I included only what they asked for, Proof of the last 5 years i used bank statements, i included my immunization records, Passport. I didn't do anything on june 15 so i submitted that months bank statement. I didn't include any awards or certificates. I didn't have proof of where i have lived since i got here, Only the last 8 years. The lawyer just told me to include the street...i was like really i can be that vague. Anyways i got approved.
