Renewal question

1. On form i-765 quesiton 16, who may file i-765 ? what did you guys put ? this is for renewal ? I think C33 ? but there are like 3 blanks ?
2. and also I'm already late for the 4-5 month renewal period is this Ok? my EAD expires on 10/18 so I should apply between 05/18 - 06/18 but the forms isn't available until 06/05 so that would only give me 13 days to renewal.

Mine expires Oct. 1st and I sent mine in. Send yours in as soon as you can using the correct form. Yes, its C33

It gives you 13 days plus the rest of the 4 months to renew. It's advised to renew as soon as you can.
