form I-821D Additional Information Part

Did you guys leave this part completely blank if you didn't have anything additional to add? Or did you put your name and sign it anyways?

I filled out my app on the computer and I believe it copied my name and A-number to that page. So I have it there but I didn't sign it because there's no relevant information on the page.
