renewal form 1-821d changes? do i have the right form?

so I'm getting ready to send my forms back when i noticed a difference between 1-821 forms. i have an old copy of my first renewal which happens to be the same form since then form i-821d OMB No 1615-021 expires on 6/30/2016 edition 06/04/14. the difference i spotted between the two is that the old one page 7 in part 8 it has a 6)a and 6)b section for a signature and date but the one i download straight from the uscis website which happens to be the same doesn't have those two sections instead the app ends with part 8 section 5)D. do i have the wrong application version or is everyone else's application end with part 8 section 5)D


also do we do anything for part 8 if we dont need the extra space or do we just fill out the legal name part and A-number and leave everything else blank
