Question About Washington License

Hi, I currently lost my Washington license...
What are the proper documents I need in order to obtain a new one?
I have a foreign passport which would equate to an "A LIST Doc" It IS valid but i'm not positive that it is stamped by the US customs and Border Protection.

For my "B-list" Docs I have either a school transcript that includes my DOB and my Social security card.

Also- will I need to provide proof that I live at this address if i am not applying for a NEW license?

If you areleady were issued one you just go and get one mailed at the address you provided. You only have to provide everything the first time you were issued one.

The first time I went I have a Georgia license.. and obviously I gave that one to them already. so this time I only have my passport... is that ok?
