hmm i'm a little worried! i went to my old HS last week to ask for my school transcript and they said they could could only give me an UNOFFICIAL copy of my records (they send OFFICIAL papers to colleges ONLY) So I asked them what exactly was the diff between official and unoffical papers and they told me it was the "seal" that made it official, everything else was the same. So when my unofficial transcript came in the mail, all i got was ONE piece of paper with my grades and all the classes i took all 4 yrs of HS. Is this how transcripts are supposed to look?? should i be concerned?

My is a stamped copy of my transcript. It is 3 pages:

page 1: My grades classes and grades including my school rank and TAKS score (I am from Texas)
Page 2: More Standardized score reports and my SAT score summary
Page 3: My AP (Advanced placement) test scores

All pages have a stamp from my school district registrar certifying that it is a correct copy of the original transcript.

Your high school keeps records of immunizations. They should've given you a copy of your immunization records upon graduating or days prior to it. You can ask your high school how you can obtain a copy of your immunization records.

In Chicago, all you have to do is go to http://www.cps.edu/About_CPS/Departm...ntRecords.aspx and you can request HS transcripts and immunization records to be mailed to your house.

my ONE-page transcript has a stamp that says unofficial transcript on it!

well I'm not really that concerned that my transcript doesnt contain any SAT and AP information. i have official copies of those myself so no problem there. The fact that I got into college is sufficient proof that i did well on my SATs.

Sorry, I really didn't read your entire post but have you thought about going to your school district's office and request a transcript and not necessarily to your HS? Maybe there they can give you an official transcript.
Where I live, I had to go to a small building that is close to the district's office and they have all the school buses and stuff and that is where I got my transcript.
I'm sure your school district or city has something similar.

In NYC, the official transcript should have a school stamp on it and sealed in an envelope.

It does not include any medical record.

As for medical record from your doctor's office, the staff will ask you to sign a release form and allow 10 days for review by laws.

Some places will charge you 75 cents per page by law.

From a specialist, you need to ask your current family doctor to fax them a release form.
