"What Documents May Demonstrate That You Were Present in the United States during the 5-year period immediately Before June 15, 2012 and up to the present date?"

a. Rent receipts, utility bills, receipts or letters form companies showing the dates during which you received services.

Question: Do I need to provide utility bills for every single month? And for which period does that that need to be covered? June 15, 2007 - Aug 14, 2012?

Employment records, a good idea? since we arent suppose to be working in the first place, wouldn't that affect our Work permit when we apply for one ?

I had a question about this too. I assume this includes college education and not just high school/GED education?

I'm not totally sure what to put here, I've never filled out anything that requires an SSN but what if my parents did before with my ITIN?

I used to volunteer at a hospital and we had to sign in and sign out. I called the volunteer services department last week to get a log of my hours to use as proof of continual presence. They sent it to me as a letter thanking me for my service.

If your clinic makes you "sign in" or log in, I believe the official record would count the same as a receipt.

Yes, all documentation in another language must be translated to english. USCIS used to have a 'any bilingual person can certify the translation is accurate' clause on their website, however the guidelines for this form indicate you may need to have someone who is 'certified'. quick answer, dunno if anyone can do it for u.

hmm, I don't think you can put present on there can you? What about date when semester ends?

I think for the june 15 presence deal you may not use an affidavit, but you may use circumstantial evidence (ie receipts, pictures, etc...). Someone may want to chime in here.
