Well, if you look at Form I-821D, the top right corner says "Expires 2/28/2013", so I guess the deadline is February 28, 2013

Put exactly like this "WI - Without Inspection"

the form is a pdf and has scroll down menu's on some answers. That one is the one that fits most to yours.
