I believe you leave both blank, the USCIS office, has to do with the results, granted or denied part also.

help guys, what does "Currently in proceedings mean?" Does it mean i'm being deported as i speak? Or should i put "Subject to a final order"?
I was given voluntary departure as far as we knew....

It's (C)(33). The instruction for I-765 says we should put that in for question 16, as categorized thru deffered action (DACA). It also says put "Unlawful Status" for question 15.

BTW...what is our Alien Number??? Should we just leave it blank if we don't have it??

I'm not sure how somebody here would have an Alien Number, but I know that for example my dad had one when he was being requested by his job. It's number of the form A123456789 that they give immigrants when they are adjusting/have adjusted their immigration status (I think you even keep it after you get a green card). I've read that you also likely have an Alien number if you been in removal proceedings or pretty much if you've had any direct interaction with them as far as immigration goes.

Guys. I came here on a valid passport with visa, but my last name in it is my Aunt's last name. However, I've been using my parents last name since living here. Should I include my passport details in the application and just put my passport name under "other names used"?

Just a very simple question: Under the "address" portion of these forms it gives the option of three check boxes; 1. Apt.; 2. Ste.; 3. Flr.; If we live in a house and none of those are obviously applicable, do we just not check any of them?
