No problem. That's just a sample, and they guy who made that packet was probably filing for "I-601 Grounds of Excludability", that's why it's in the cover page. Ours, I assume, would be something like "I-821D: Consideration for DACA+ I-765: Application for EAD".
If in doubt, just label your cover page with what you're applying for.

is anyone listing the address of the out of state driver's license on the forms???
I was thinking of not listing it....

Ok I have some more questions: just called USCIS and they gave meno info they prettymuch sent me to the instructions or an attourny but they confirm the money order or checks can be just one for the total amount.. so here we go:

on I-821D: if i still in School do i put any date in question 20?

in I-765 (c)(33) if the correct code? i think it is but lot of post made me confused. I havent be sent anything by ICE ever... so.. still c33? ot c14?

If we are EWI we still put "(c)(33)" on the I-765?

Assuming you're still in high school, and that you're in summer vacation, put the last day you were in school. Like the last day of school for the past school year.

For i-765 the correct code is (c)(33)

You don't neeeeed a cover letter, but it will help the person reviewing your application know exactly what he/she is looking at.
