Yes, the code "(c)(33)" applies to everyone applying for DACA, regardless of manner/status of entry

For i-765 the correct code is (c)(33)

You don't neeeeed a cover letter, but it will help the person reviewing your application know exactly what he/she is looking at.
OMG this has been the best reply i have got!! THANKS!!!

Check this graph posted by USCIS to know where to file

http://www.uscis.gov/portal/site/usc...0045f3d6a1RCRD

And yes, you do include the check and copies as part of your application packet

Are you guys filling out the application via type? If so, are you using all capital letters? It's known not to USE THEM WHEN TALKING ONLINE for example, but filling out the forms seems like it would be more readable/clearer for some reason....

Hopefully someone can help me with this!
For question 16 a, it asks for the 1-94 form. Well what if you have the previous version of it, the i-444? Should I write it in anyway? I called already and they weren't too sure about it so I'm looking for other options.
Please help

Can anyone tell me if you have to prove presence in the US for every month in the last 5 years or will one piece of proof per year suffice?

I first came into the U.S. at 3 months old and have proof of me having immunization records, but when i was 5 years my parents and I went to Mexico and came back to the U.S. a year later. For the form i-821 question 13 would i put the date when I was 3 months old or when I was 6 years old?(Ever since I was 6 I haven't left the U.S.) Also for the I-765 which one would I put? I'll appreciate if anyone can help.

You put the name of the airport:
Example:
John F Kennedy International Airport, NY

2 separate enotification forms or just 1? Since there is no fee, then I don't see the harm in sending 2.
I will be sending 2[/quote]

"If you are submitting multiple immigration forms for one applicant, clip a completed form G-1145 to the front of the first immigration form of the package. You will receive a separate e-mail and/or text message for each accepted immigration form.

For representatives who file multiple unrelated immigration forms in one envelope, and who want their clients to receive e-Notification, clip a completed form G-1145 for each applicant to the front of their immigration forms package. One e-mail and/or text message will be sent per accepted immigration form, to the person seeking the benefit(s) (representatives are not copied)."

This is my ordeal.
On June 15, 2012 I went to get my Matricula Consular. It has the issue date on that day and at the bottom under Authority it has "ConsulMex San Antonio". Should that be enough proof I was here in the US since I can't find the receipt?
