?? What about second last name??

HEY GUYS, some of us, on our passports appear to have both our paternal and maternal last names.
Should we include both?
How are you guys going about this?
Is our maternal last name the maiden name?

Use what shows up on your passport as your last name. Both paternal and maternal.

No, your maternal last name is not a maiden name. A maiden name is the name an unmarried woman gives up once she is married.

Maiden name is for women who are married and changed their names legally.

Not sure about the maternal/paternal thing. I have two last names, but I'm going with the one that is stated in my passport (my lawyer capitalized BOTH names and had them in the last name box)

I have the same problem, my foreign stuff like passport and birth certificate have both names, but my school records and bank stuff has only one, they dont even have my middle name so nothing matches. I think that in the first part I will put my full name as it appears in my foreign stuff and for the part that asks what other names you use I will put the name as it appears on my U.S. stuff. What do you think?

Thank you guys for the info. and Dreamer3000 yeah that sounds great. On the first both the complete name as it appears on the passport and on the "other names used box" the ones used for other documents. GOOD JOB PPLZ
