I have two transactions in my bank statements very close to the june 15th date ( one in june11, and 19th) would that be sufficient evident to prove that I was here that date?

Yes, as closest to the date as possible.

If on your birth certificate it has your both of your parents last names, but on my high school diploma and other paperwork it only includes my fathers last name do I include both or just my father's last name?

It should be good enough, but I suggest you to find as close as possible

Both. Whatever is on your birth certificate is technically your legal name. Under additional information/other names used, write down that your mother's name is sometimes left out of documents (ie high school transcripts)
