Quick question:

We dont have to fill out parts 5 and 6 of I-821D(preparer's information) right ( if we are filling it out ourselves)??
Only double checking since there's no where else on the form where our email address is requested.

so...does anyone have a out of state license?? Are you listing the address of your out of state license on the application? I just went to a place where they are helping people fill out applications and they said we should list ALL addresses, specially if it's on a driver's license, since it will come up during a background check...
Any thoughts??

No, you leave those two parts blank if you don't have someone else filling or interpreting the forms for you.
You would put your email address on the form G-1145 (if you choose to submit it).

She put everything else the way you have it in the reply example and she signed it but in the space where it says address she just put Los Angeles, CA.There also wasn't any type of official seal or anything, just her signature. wtf! is this cool? or did she forget and im screwed for not mentioning it??

it should only say that she speaks English and name and signature, I have translated birth certificated before for uscis related purposes. uscis just wants you to have your birth certificate translated by someoneone else other than yourself or family relative.

Today I went to get my birth certificate translated from spanish to english by a notary public for 75 dollars since I have an expired passport. Did I get ripped off or is that a decent price?

That sounds pretty steep. I paid $25 for mine.

Pricey. How much is it to renew or get a new passport from your country?

Someone please answer this.
Do I send all my bank statements for the year or just a few?
Is every month to be accounted for or just the years?
