I called the uscis information number yesterday and this is what they told me:
-The birth certificate should be translated by a certified translator with a seal ( not necessarily notarized)
-On family name, write whats on your birth certificate, which in my case would be my fathers and mothers.

Just thought I should share.

checking boxes, we mark an X right?

And for address portions (eg. Page 1, 2abc) of I821 form, if I live in an apt, do I write

ex.
2b. 123 Fake Street
2c. X next to Apt and Apt unit number on that box after Ste. Flr.?

On for I 821d, Page 1 Question 3d. I signed a voluntary departure order. Do I put the information of the date I was placed in removal proceedings or the date I signed the voluntary departure?

If I am doing this app by myself do I leave Requestor's Certification section blank?And leave page 5 of (I821 app) completely blank?

I just put unlawful status:daca. Yes we mark x. Yes on the address.

This is my take on question 15 regarding current status in form i765. On the instructions it says that once you're granted Deferred Action THEN they'll take a look at your i765. To me that means that although your status at the time of filling is Unlawful, by the time they look at the i765 it will be .DACA

Thank you guys for those clarifications!

I am not sure whether I am EWI or TWOV for question 14 on I765. I came here illegally as a child through the airport.

And are checks instead of money orders ok?

Do we have to prove our expenses and income for I765 with documents?
