I know people have answered questions as to whether we should mail one or two separate checks/money orders (one for the $85 and the other for the $380), but I found this link which gives step by step advice to fill out the application and it says to send two different ones. I already bought my money order for $465, but I don't think it will be an issue. Right? Right?! Someone please calm my anxiety. He he. Anyway here's the link:

-does anyone know if i can send my birth certificate with out translating it if i have a valid Passport (it's renewed)?
and is it still valid if it doesn't have a visa for the u.s.?

Okay so now, on the i-765 application, there's also another place to put other names used, but the space is not big enough for all my name variations. So, should I leave it blank? Or should I just put it then 1 name variation I use the most? I mean I usually sign my name with my first and last only. Even though it's 4 names long. So... ??? Argh! I hate having such a long name. Also, I have been filling it out the way it is on my passport, so under where it usually say middle name I'm writing both my actual middle name + mom's last name. Is that alright?

It's been confirmed (you can search the forums for it) that you can send one OR two. You're fine.

Hello.
First and foremost I would like to thank everyone here, first those answering questions and then those who are asking them. Looking through this forum I have had a lot of my questions and have gotten everything straighten out for the most part, except for the proof of continous residence in the United States.
My problem is that I don't work nor have I gone to a school in the last year and a half. Basically this whole year has been one empty void for me as far as receipts or anything bearning my signature or information. I do all my legal shenanigans ( I am on a petition through my citizen parents) bank account, online orders and such through my parents so I dont have any documents that is continuously been sent to me that has my name and information on it.
I would like to know if the Tax application forms can be used as proof. I have also been a memeber and a musician to a church since I was in my teens. I dont have any documents from that because I dont get paid for playing but could the pastor of my church weigh in and give me an affidavit that he know that I have not left the country since he knows me?

Yes! It's a requirement. It tells you so on the i-765 instructions. Also on Step 4 on here:

On page 6 of the I-765 instructions, "Required Documentation"
