That is correct, you don't necessarily need something dated on June 15th. As long as you have evidence that places you in the U.S. before and after June 15th.

This threw me off a little. How do we provide evidence that we entered USA without inspection?

I don't know maybe I'm reading too much into it, because at first I thought well its just asking for anything that proves I was here before July 12,2012. I need someone elses point of view on this one please.

^ I just graduated with a Masters and that's what I put. Then under additional information page I also gave my high school name/graduation year and undergrad name/graduation year since it wasn't clear what they wanted for that question.

Are you guys sending bank statements for EVERY month? Would't that be too much? I think one every two or three months would suffice. Thoughts on this?

does the letter of "economic hardship" goes in the bottom of that form ?

The form itself says that you don't need proofs, but they will review it if you provide one. Obviously, dreamers like us will not have a problem proving our economic necessity since we've lived in a tight financial situation, but be creative with your writing. If you had to write a boo-hoo story to strengthen your case--make sure it's not a fake story tho, jsut make the real one sound good--so be it.
