For I-821D part 1 #15 "Status of Entry" it would be without inspection if I didn't come to the U.S. with a visa correct, rather through Canada, correct?

for #19 "Education Information" date of last attendance of school, what do I put if I am currently in school? Today's date or blank? I read that if it doesn't apply you leave it blank?

Do you guys think I should wait to get an official gov ID before sending my packet in or will birth certificates and school ID's work? I haven't renewed my passport in years and I know the wait will be long at the consulate.

Yes those two are fine as said in the instructions! Also send copies of the expired passport.

Well, I was confused as to the wording, because I did live in Canada after my initial entry into the US, but it does make sense that they would not be interested in a foreign address.

I visited them from the US. I came back to the states after living in Canada, and visited my relatives in China.

The concerning Residence is that the Dreamer in question hasn't left the US for more than 90 days straight so a brief 3 weeks would be fine.

If you didn't come to the US with your own visa and passport I believe it has been mentioned you'd have to place EWI on the form.

If you filled out the first question as still in school you can leave it blank.
