Confused on filling the I-765 and I-765WS

I have searched around the forum and I could not find a concrete answer.

So on the I-765 form, there are a couple questions I am confused with.
I have read the instructions numerous times, in attempt to depict the pieces of information that I needed.

For number 14 on form I-765, "Manner of Entry", if I came at a very young age on a visa (around 2 or 3 years old), would I just put visitor? (The visa is now expired by the way.)

For number 15, do I put "undocumented," do I put "student," or do I put the combination of letters and numbers [e.g. (c)(33)? am going into my senior year of high school, by the way.

Also for number 16, do I put (c)(33)? Is that the number we use for DACA applicants? Sorry I am asking for so much. I just want everything to be correct.

for 14 write visitor if you came with a tourist visa.
for 15 put "Unlawful Status".
for 16 put (c)(33)( ).
