Ok let's try to think through this logically.

Two of the requirements deal with your age. So you have to find a way to prove your date of birth. The two documents that are best for this are 1) the birth certificate and 2) passport.
if you have neither, how else are you gonna prove your age? The matricula is generally not seen very highly.

my answer: Either get a valid passport or get a translated birth certificate and some other form of ID (matricula should work)

I think the general consensus has been to not include fake SSN's.

Doesn't the application specifically ask if you've every applied for a work permit before? and then asks for some information? How would you know those details if you don't have the forms? And if you do have the forms, why don't you just make a copy and send them?

1. USCIS prefers that you not staple anything. they have to scan every piece of papers, so staples are a pain in the ass. use paperclips
2. I just mailed it in the envelope. it should be fine.
3. No, that shouldn't be an issue. as long as the money is there
4. You need to prove you were here between june 15th 2007 and 2008. You might have issues if you can't find documents to fill that gap. 2004-2007 shouldn't matter though.

I keep getting all types of answers on here, but the payment is only ONE check or money order correct? Some people say TWO because the I-821D is $380 and biometrics which is for the EAD is $85, but you can get denied for EAD, so the $85 is theirs after cashing the $465 check?

One check/money order of $465 should be fine. Many people have called the USCIS and spoke with a representative that said it does not matter if it's one check or two. Your choice. Plus I would assume we are all required to go through biometrics before being accepted/rejected for an EAD anyway. So regardless you have to pay that amount no matter what? my guess? But anyway, point is... One check/money order of $465 or two separate checks/money orders of $85 and $380 is fine. Your choice.
