Thanks everyone for the help I really appreciate it. I just have one final question on the 1-765 worksheet it says to write down my current annual income. Should I put down what i made last year or what I have made this year so far.

Last year I made a little over 20,000 but I had way more bills, since then I was let go by the company.

Calculate your CURRENT annual income using your hourly rate and your hours per week.

In the lines provided Give an explanation of it.
If you plan to include your tax returns for the 2011 you should hire a lawyer to take care of this form for you.

Is there a reason why? I spoke to my attorney and he said that if you are submitting copies of information, then you can highlight stuff on it. He mentioned that almost everything is kind of based on an honor code...people can fraud almost every document youre submitting with the exception of some that include numbers (SSN, A#) which can be looked up.

Ok so I've been doing some reading my question is regarding form 1-765 question 9 "Social Security Number (include any number you have ever used)" I used one i made up should I write it down?? I did my taxes using my ITIN number.

Should I just shell out the money to talk to a lawyer?

Yes write down the one that you made up... If you worked a SSN under your name, note it down for that question, your employer makes a tax report towards the SSN you provided.

DO NOT STAPLE!!!!
these paperwork are processed by machines - all paper work is scanned by a machine and originals discarded or locked up somewhere. Machines don't deal well with staples. USE PAPERCLIPS! (employees there will probably remove the staples before feeding it but make it easier for them and prevent discouraged workers from having to rip your application while trying to get rid of staples.) Put your money order / check first - your email/text confirmation form - your I-821D form - your passport size photo - employment application - supporting letter for employment application - all your evidence. DO NOT STAPLE a simple paperclip is more then enough.

Good luck

Here are some FILING TIPS provided by the US DHS
for filing I-821 ... which is basically the root of I-821D
