For the under 16 entry requirement, I sent a copy of my i-94. I also sent my school records, dating back to elementary. Then my middle school, high school, college transcripts. I sent a bank statement that covers from 2011-present. I wasn't able to cover the summer gaps (since I have no school at those times).

WOW! i got the I-765 APPROVED!!! but not the I-821D, it hasnt been updated yet! But the I-765 is the most important one right?

So here is what i sent:
1 passport/consulate id/birth certificate + translation for proof of identity and age (31+ here btw was 30 on june 15)
2 school record from 1-12th (big packet! had to add 6-8 grade, grades) for proof i got here before 16, included is transcript with credits i completed for graduation
3 H.S. Diploma and honor roll certificate for proof of education
4. june 15th proof was bank statements from june - july, which had transaction june 11, june 23rd, june 9th, copy of ticket stub that i rode train june 9 going, and june 19th back
5. sent arrest records i got from sheriffs
6. 5 years of residency, used bank statements june - july 2012 and immunization record for a feb 2012 date, selective service card, copy of itin to show its my number
07,08,10,11 i used my tax return, thats it, quiet a bit packet though. For 09 i got bank statements from jan, april, aug, and dec, also copies of money orders i bought and obviously i was EWI

Congratulations! You guys don't have to worry. Yesterday I got four texts. The first one was for I765 saying my card is in production, the second was the approval for I821 and then the third one was for I765 post decision activity. I got worried because I was like omg what happened to my card production??! Then four hours later I got another text and it's for I765 saying my card is in production again. lol
