I explained that I was applying for DACA and work authorization, she asked to see the receipt notice for the work authorization, after I showed to her she stamped that letter and signed it along with the bio one and she told me that if I got a letter requesting me to go to another bio app for the work authorization to ignore it. I'm sure you'll be fine if you only put one down, everyone and every service center seems to be doing things differently. Good luck!!

The lady that gave me my form filled that part out and I believe she only wrote down one number. Although, I did notice she made a mistake while writing down I-821D so she crossed it out and wrote over it. I was so paranoid whoever got my form would not understand what it said. Obviously, it was no big deal.
