Residency affidavit question(Texas)

I need to fill out a residency affidavit for the college I am applying to, and for that I need to fill this out and send it oit. ----->

My question is can I do it myself, or do I have to get a notary to do it? Thanks in advance.

i've had to fill that out a few times (for both undergrad and grad school)...i took it to Chase since i bank there. They have notary services. you can either fill out #1&4 and take it in..or print and fill it out with the notary present
