Apply with your real name. But under names used write the names you have used. Just be homest bc uscis will deny u if ur not honest. My sis had the same case. She was honest and got her permit

This is a tough one. I suggest you get a lawyer to help you with this, Because i would think that the moment they get your bios, your old visa and name will pop out. They might see it a u trying to commit fraud, which you did, but without your knowledge. So in another case, you could afford to get denied and then fight them to prove it was not you, and u had no knowledge of this. Sadly with DACA its either or, with no 2nd chance. Like i said get a lawyer see what he says, he might make a letter explaining how it all happened. Could work who knows. Worst case is; You have to skip DACA and hope for DREAM ACT ,CIR or some kind or immigration law to pass later this year, that allows you to contend.
