I-765 Worksheet (PDF) (when filed concurrently with the I-821D)

12/11/12-Application mailed to Chicago
12/14/12-Email recieved fom uscis saying my application has been transferred to Texas.
12/19/12 - Recieved receipts
02/06/13- Biometrics Appointment
03/19/13- Email notification saying Employment authorization approved
