Question # 12 Date of Last Entry

Hi guys!

I am filling out my DACA documents tonight;
However, on question # 12 of I-765, it asks for the date of last entry to the US.
I came to the USA when I was 6 years old, in July of 2001 with a tourist visa.
I do not have the exact day, just month and year.
What should I enter?
Any help would be deeply appreciated.

You don't have a stamp on the visa or an I-94 with the date stamped? It should be on there I think. If not month and year will have to do.

Yes, there should be a stamp on your passport with date of entry. I made a copy of my photo/personal information from my passport, copy of the visa/stamp and i94 and mailed everything with my DACA application to be safe. I don't know if it's necessary but it can't hurt as I was approved only 2 months later. Good luck!
