got rfe. lost rfe envelope. need rfe address for nebraska

Hi fellow dreamers. Long story short I got an rfe and misplaced the envelope. I still have rfe letter. I do remember the envelope was destined to Nebraska. Can anybody help me in obtaining the address I need to send it to. ?

The RFE address should be at the bottom of the RFE letter.
