That is the link for the form

You could state a purpose but its not really required, you could also include your receipt number where it indicates it if you are only interested in knowing about that one particular case or you can leave it blank and just include your A# and let them send you everything they have for you.

sign it and mail to :

Must we provide a signature under a Notarized Affidavit on the form or is this for something else? I'll try anything at this point just to find out what is taking forever.

You can do a FOIA but it won't give you any information on current cases until after adjudication. It is possible that filing a FOIA will "speed up" your case in a way or at least that's what I'm hoping. I did a request not too long ago so who knows what can happen.

I have a friend who wants to fill our this form to find out the exact date of this visa entry to the US. almost fifteen years ago... He does not have his A# nor his original passport because he lost it a long time ago..

Does anyone know if he can still fill it out, without having any issues, or does any one know if immigration will come out to find him because of his overstayed expired visa.?
