Under my current immigration status, I can not work legally thus earn income at all. My yearly expenses are based off food, cell phone, school supplies, extracurricular activities and miscellaneous expenses. My parents try to help me out as much as they can and I pay my extracurricular activities through fund raisers organized by different organizations. With this work authorization I would have an amazing opportunity to acquire some much needed money to pay for my expenses. I also plan to attend college in the near future and this work authorization would definitely help pay for a tuition. If given this opportunity to work i will take full advantage my skills and future career.

Looks fine to me. Just keep in mind this is strictly professional. So you don't have to write like emotional things or an essay describing how good of an opportunity it would be if they approved you. The point is simple. Just say you have this and this expense like you mentioned. That's all thats needed. If you meet all the eligibility requirements for DACA and show full proof documentation for this, you will have your work permit. This extra work sheet is just a supplementary form they are required to have in file for you since you're applying for a work permit. It should be the least of your concern but yes do fill it out. Make it short. No essays and no emotional or personal things. You're paying a pretty big fee and you meet all the guidelines, thats all you need. Good luck.
