Question for change of address for renewal

hey guys,
I got my EAD 10/29/2012 and I have moved 06/30/2013. My EAD will be expired this Oct so I am preparing for the renewal process.
I have a question, since I moved, do I need to attach any proof of address with the renewal forms?

Did you complete the change of address form on the USCIS website? I believe it's Ar-11. It's completely online, and you don't need to send it in.

Mine expires on 9/26/14, and I changed my address in May.
I filed the AR-11 online, and I printed it out to show that I notified USCIS of the change and attached it to my I-821D just in case.

I don't remember, but I think I did change my address via USCIS web back then when I moved.
Is there any way I can verify?

Is it OK to attach my proof of address with my mail and put a note saying this is my new address something like that?
