This is what I have right now.

i-821D
i-765
i-765ws
G-1145
2 passport photos
$465 check made to U.S Department of Homeland Security
copy of ead card

Anything else? Any tips on how to assemble this package? Where should I send it to? I'm in Texas, btw. Sorry guys first time I do this.
-Use acco clips instead of paper clips. DO NOT STAPLE!
-Write your Full name and A# on the envelope you use.
-And then put that envelope in the flat rate express mail envelope provided at the post office. Good luck.

Correct order:
G-1145
$465 check made to U.S Department of Homeland Security
2 passport photos {just so it doesn't fall off the paper clip in the back}
i-821D
i-765
i-765ws
copy of ead card

That is not required. However, I did include it in my packet because it contains my A-number.

Thank you guys, y'all are awesome!
This 2 are my last questions and kinda dumb question but rather ask.. the acco clips you mentioned, do I use just one for the entire package or I could use 2? One for the entire package and another for the form i-821d(it's multiple pages). And should I put the passport photos in a small envelope or not?

Will have to send it on Monday as USPS is closed by now

I kinda had forgotten about this renewal but received an email this week from HR saying my work permit was expiring in October and I needed to show work authorization after that.

Did you all answer #8,9a,9b,9b,9c, under travel information in the i821D application?

Use a small paper clip for the I-821D forms and use an ACCO clip to stack the whole package together.
And I used the envelope style paper card that I got the passport photos in when I got them done at CVS, you could just use that or u can use an envelope that's cool too.

Oh and don't forget to put your name and A# on the back of the photos as well, using a pencil preferably.

Even if I haven't traveled outside the U.S. at any time?

I left that blank, since I haven't gone anywhere since I came here as a kiddo. And checked the "NO" box where it says "Have you left the US without advance parole on or after Aug 15, 2015?

Thanks for the input Smarty! That's what I had originally assumed.
I will be sending my application on Monday, at this point I'm just double checking everything.

I checked no on 8.. but answered 9a, 9b, and 9c. I think you should! 6a to 7c are the ones I left blank since I didn't go anywhere. Oh and 10 since I don't have a border crossing card.
