I used a 10" x 13" envelope because i included some tags on the side of each page... but any size will be fine

Hi I have just filled out my forms which include i765, i765 worksheet, and i821d. Do I need to make 2 separate checks, one for $380 and one for $85 made out t o U.S. Department of Homeland Security? Also how many passport pics should I include when I mail out my application and what is the address for the Chicago Lockbox?

i think you only need one check for the amount of 465, and 2 passport pictures are needed.
