OP has yet to say whether they signed part 5 or not.

They need to sign part 5, 1b

If you prepared it, the only part you need to sign is Part 5 2a. If you filled out page 8 with Additional Info, you would also need to sign that. As well as signature for I-765.

So the right thing is: Sign part 5 2.a and part 8 6.a ?????

so evverybody still needs to fill part 8 where it ask for name,last and alien number right??? And leave 6a n/a and no date also?

I left everything on the last page blank. It just looks like OP didn't sign part 5. Be more careful, people!

I had additional information to add, so I filled out part 8 with my signature. It can't hurt to put that information with signature even if left blank, I suppose. (Just make sure you sign part 5 2a as well!) I left page 6 completely blank. I did not fill in n/a and my application was accepted.

It appears OP forgot to sign part 5 and that is the cause of rejection.

Okay guys thank you all for your help.. turns out that i totally missed part 5 and went straight to the end and signed the part..

i had to print out a whole new application went through it a billion times and also had a friend looked at it which was really helpful too.

wait so you have to repay again?

i send back the same money order but with a new printed form
