Daca renewal help

So the lawyer I used my first time to apply for daca does not want to give me a copy of the old daca paper work and I'm not really sure what dates he used for my first application.. I know the month and year but I don't remember the exact days . He said if I need a copy of my daca that I need to contact uscis so they can send me a copy and that it will take time ! Which I don't have because my 120 days start this week Any advise?? Also do I need to put my name on section number 8 of the I821D ?!

So when i applied I used one of my last names only on all my applications example : Maria Suarez but USCIS changed my work permit to my full name on my birth certificate Maria Suarez Mendez. SHould I apply with the second full last name since i know im suppose to write the same thing i wrote in my first application WHich was Maria Suarez?

You should write it the way it is now .. That happened to me as well and now I have both last names my dads and moms
