Help! I-765 Questions

Hello good folks

Quick questions for form I-765 and I-765ws:

Question 11, the answer of course is yes (renewal) but what do we put for the following "which USCIS office?" and "date"?


Question 16 is it still code (c) (33) ()?

On form I-765ws, is an "explanation" really necessary? What do you guys think?


Also, I have read conflicting information and weather or not to include the 2 passport pictures, does anyone know whats up with that? I included them in the initial application.

Question 11:
Which USCIS Office?
- is on your approval letters... it could be: california service center, nebraska service center, vermont service center, etc.. look on the bottom left corner of the approval notice
Date?
-Your approval date or the Valid From Date on your EAD

Im not really sure if the explanation on the i765ws is really necessary. I included an explanation just to make sure i dont get rejected, but some people didnt include it...
And the same with the passport pictures.. some included it and some not... this time i sent the passport pictures... in my initial application i didnt...
