It is a requirement for DACA and renewal. Otherwise, You wouldn't be sending in the worksheet.
And in OP's case, his EAD app got RFE'd. Why would a DACA case be approved but his EAD not? There is a reason why we have to prove economic necessity for employment authorization, how can USCIS issue an EAd without interpreting the alien's economic necessity under immigration law

BTW working illegally does not disqualify you for DACA or EAD, people using fake SSN have been approved here. They are NOT the IRS don't forget that.

i do not have the copies, the copies are over at the church that helped me fill the whole thing out, and so the RFE will get to them first most likely.

Mistake from the church? Ucis? I mean I doubt the church made a mistake, they had lawyers either fill the papers with you or checking them before sending them, so, I doubt it.

Id honestly would go get them and see what they filled in for you

i really should be considered the worlds most stupid man. I forgot to submit my 2 passport, color pictures!!!! i should have had my EAD BY NOW! i am so mad at myself

So why didnt the church tell you to add them?
