Question about form i-765 for renewal

Hi everyone, my daca will expire on 04/11/2015 and I am planning on sending my renewal package this week. On Form I-765, question 11 where it asks for Which USCIS office approved my daca, i don't know what to put in there. Do I put the office where I did my fingerprints or do i put the office that I sent my package to? Thanks.

Edit: Also, question 15 asks for Current Immigration Status, do I put No Lawful Status?

On question 15, do I put DACA Approved?

That's fine. I put DACA Recipient. Like most people I am C33 category so they will figure out (plus you submit DACA form and requested Renewal).

Or just put DACA

Also, I think it's a little late sending yours on the fourth month before expiration. DACA clearly states that it must be at least 5 months prior. It also depends on your packet's arrival, an extra week of delay before it actually arrives to office. Just my take.
