Lost EAD card

I am in the DACA program.
to replace my ead card which forms do i need to fill out again?

i765 form with payment

do i have to send an i-821 form with pictures?
&how long would it take if you know.

821 DACA application form is not required if you are only requesting EAD lost card replacement. Photos will be required, check 765 instructions guide for full details. I've read it takes approximately 3 months for replacement on this forum. May be longer, not certain since I've never lost one.
