This is the website to file a request. You should tell them how the case has affected you (like financial hardship, etc.) And ask them to expedite the case. So far they haven't rejected my brother's request yet. They told me that they sent the request to an analyst who works specifically with DACA applicants.
I also received an email today basically stating: the case has been pending for 111 days. USCIS's timeframe is 120 days and If we haven't gotten a decision within 9 days to email the analyst directly.
I highly recommend that ones you file your request to email them checking up on the request otherwise they will keep you waiting. I followed up on them 3x till the analyst finally just emailed us directly.

one for i821d and one for i765.
the i765 receipt number shows if your card is in production.
