AOS after AP and DACA

I would like for someone to please put a summary of what they sent on the package for AOS after they have received the AP and DACA.

Did you sent the I-130 and I-485 with all documents requested or did you send them at different times. If someone could please put up a quick summary of what they sent out would be of great help and what they put in for question 14 where it asked about the last entry ? did you just put in the information from the I-94?

Basically what all did the package you sent included all forms and documents would be of great help.
