1. In Part 2, Section 6a, "Travel Info" what do we put if we never EVER left the united states? Do we leave it blank or put "N/A" in the boxes?

I was looking at my renewal paperwork from 2 yrs ago and I left that blank.

2. In Part 8, Section 1a, Do I put my Legal Name, and A-Number again? I don't need to put any additional info. Do we leave this blank or put N/A again?

The PDF form automatically filled that info in for me... and I did not include any additional info. So just put your name and A-# again.

3. Part 6, Part 7, do we leave these areas blank or write N/A? I am the only one completing this document.

Again, looking at my renewal forms from 2 years ago, I left these areas blank.

Form I-765

1. This is my second renewal so I have been granted an EAD for 4 years now. Part 11 asks if I've ever applied for EAD from USCIS. What dates should I put here? Just the recent renewal, or the renewal plus the initial application?

Since it only has room for 1 date, I put my renewal date from 2 yrs ago.

Also which office should be written on it. The one that granted my request correct?

Yes, the one that granted ur request.

2. Part 15, What do we write under Current Immigration Status? Just DACA?

Yes, but spell it out, Deferred Action Childhood Arrival.


Filing

1. Do I write two separate checks, or just one? Just 1 is fine

2. Cover Page needed? I didn't include a cover page but I did include the tear off paper that came with my old EAD. The one that has my A #, Address, Receipt # with barcode, Card # with barcode and some other barcode is on there.

3. Any certain order the packet is supposed to be filed in?


This is what I was wondering too.....

This is how I assembled mine:
The tear off paper with the barcodes
The filing fee in an envelope
My passport pics in an envelope
Form G-1145
Form I-821D
Form I-765
the back and front copy of my soon to expire EAD card
Form I-765WS.

I hope this is correct. I'll be mailing this in the morning so if anyone sees an error in the order please reply asap.

Do we absolutely need the passport photos? I know several ppl on the board who didn't send it in, and a personal friend who also didn't send passport photos, and they all got approved for their last renewal.
