adding spouse on u-visa need info plz

can someone point me in right direction please?
I applied for u-visa back in 2013. I got my EAD.
I would like to add my spouse in file what form should I fill ?
is it i-918 ? if so do I have to fill all my info again?
is there any fee ?

note : my spouse has DACA and he also has work permit.

If you were approved for U-1, you can apply for your spouse for a U-2 visa.

my application is approved but my u-visa is still pending. I got workpermit.
