I-821D form is experied

I looked through the forum but was not able to find an answer to my question the stickys/threads. The I-821D form on the usicis website is expired.

On the top right corner it states it expired on the 06/30/2016. I was not able to track down the current version. Where can I find the current version? I'm assuming that like other government forms as long as I'm using the most current version in their website it will be ok. I'm not worrying about it but wanted to check here before sending the packet later today. Thanks.

There is no current version available and you use that form to fill out your I-821D
