Hello everyone,

I had my application rejected at the Chicago Lockbox and returned with the following reason:

When requesting renewal of deferred action, a Form I-821D must be submitted concurrently with Form I-765. Please submit your completed package with the appropriate fees, including biometric fees, to the address listed at the bottom of this notice.

I'm at a bit of a loss, my packet included the I-765, I-765WS, I-821D, G-1145 and a check for $495. It is possible whoever processed my packet somehow missed the I-821D? There's even some serial numbers stamped on the form with initials. I'm somewhat dumbfounded, I've been through the forms everything is filled out, signed and by all accounts it's definitely all there. Anyone experience something like this before?

I'm sorry for this happening. You're the first person that I saw here that also sent to Chicago box (i'm from WA), and a bad news at that. When did you send it in? via what mail carrier?

It was delivered by USPS on 1/18. Looks like they rejected it on 1/22 and sent it back. Got it today. The only thing that's makes sense it the person who processed it somehow screwed up. The explanation is so vague I can't think it means anything other than you didn't submit an I-821D. If there way anything specifically wrong with the forms I imagine the rejection notice would have had a more detailed message.

Did you send them separate ? Look over them make sure they are the updated forms and all.

It could be a mistake. Something similar happened to my brother. USCIS claimed he didn't fill out a section when he did. I would double check all the forms and resubmit.
