Thanks for the response. Now my other question is would I be able to find this information for questions 16 - 20 online? Seeing as I already re-newed a couple of times already. Do they save this information?
I'm asking because I recently moved, and I left all the information for q's 16-20 in my other house.

Don't staple anything. If you need to, use paper clips. I just tossed everything in a manila envelope and put that inside a bigger shipping envelope.
