Sorry. Misread your question. You would write your USCIS # on the memo line. You can find it on your EAD card

How about the address line? I left mine blank and they received in Friday and has not been cashed. Kinda freaking out since one of my pages on the i821d had part of bar code missing since my printer ran out of ink. It was only really small and was wondering if they could deny my case because of that.
