i looked at the application. ITS SO simple. if you can read and write english you can fill it out yourself.

If you're getting a message that you can't see it, try saving it to your computer instead. I was able to open it and fill out most of it already. It's really very simple.

Application is simple. Read the instructions too!, they provide a lot more details as to what kind of proof you should send.

What application are you guys using to open/view the PDF file?

I had trouble viewing them with FoxIt PDF viewer but they open fine in Adobe Reader (if your getting the please wait message...).

Download Adobe Reader (for Iphone or Apple devices here).
